Early Cardio-Kidney-Metabolic (eCKM)

Hypertension, dyslipidemia, overweight/obesity with central obesity, and prediabetes. Lifestyle-led programs targeting BP, weight, lipids, and A1c.

Cardio-Kidney-Metabolic (CKM)

Diabetes, chronic kidney disease (stage 3a/3b), and atherosclerotic cardiovascular disease. Structured activity, weight, and self-management support.

Musculoskeletal (MSK)

Chronic musculoskeletal pain — back, knee, hip, shoulder. Guided movement and progressive loading with validated pain and function PROMs.

Behavioral Health (BH)

Depression and anxiety. Behavioral activation, movement, and mood tracking with PHQ / GAD instruments.
